My son, Steven, did this fro me last year. He cut the letters out with a plasma torch and tack welded the letters on the plate. It is now attached to one of my work benches.

Actually, John's pic gives me an idea that once you weld your name grind off with right angle grinder, the top to give it a flat look. Might be awesome.
